Total Brain Diagnostics (TBD): Analyzing the Impact of Functional Symptom and Clinical Measures on Co-Morbid Mental Health Conditions after TBI
Role Description: We along with our collaborators will create a unified longitudinal dataset to examine associations between physiologic biosignals/functional measures and mental health outcomes/conditions (e.g., symptoms, suicidal thoughts and behaviors). This dataset will include combined measures from the existing Long-Term Impact of Military Brain Injury (LIMBIC) and Translational Research Center for TBI and Stress Disorders (TRACTS) protocols. Initial findings from this proposed effort will be foundational for subsequent inquiry focused on enhancing diagnostic and prognostic efforts to improve brain health and psychosocial functioning among Veterans living with mTBI and mental health conditions.

Implementing Computerized Cognitive Behavioral Therapies for Suicide Prevention and Depression among At-Risk Rural and Urban Veterans
Role Description: In this program, 236 Veterans will be recruited to participate in cCBT-SP and 236 Veterans will be recruited to participate in cCBT-D. This will be accomplished using the national VHA electronic medical record to identify those at risk for suicide, a method employed by the team on multiple occasions. The cCBT-SP course is comprised of 12 online sessions and is augmented by use of a companion workbook. Sessions all have a similar structure that includes: 1) Check-In; 2) Review Previous Session; 3) Learn About New Tools; and, 4) Practice Assignment to be Completed Between Sessions. Individuals are encouraged to use the workbook to refer to plans and activities, read about skills, make notes about what was learned, and log their cCBT–SP journey. Findings from the proposed efforts are expected to identify any potential safety-related barriers to implementation (e.g., additional necessary risk management strategies, adverse events), as well as important information regarding feasibility (i.e., ease of implementation) and acceptability (i.e., suitability of an intervention from the perspective of at-risk Veterans). Such data will be vital to facilitate widespread implementation.

Recovery expectations in TBI related Outpatient Settings (RETRO): Identifying key trajectories of recovery in service members and Veterans.
Role Description: The overall study goal is to retrospectively review 20 years of data to describe characteristics of service members who sustain mild TBI. Healthcare utilization both in MHS and VHA will be analyzed to identify sub-groups and outcomes in the short-term and in the long-term.

University of Colorado - Restoring Efficient Sleep after TBI (CU-REST)
Role Description: Aim 1. Evaluate the efficacy of guided Path to Better Sleep among those with moderate to severe TBI and insomnia. Hypothesis 1. Participants randomized to Path to Better Sleep will report a significant pre- to post-intervention decrease in insomnia symptoms, as measured by the Insomnia Severity Index (ISI).60 Exploratory Aim 1. Examine discrepancies between self-reported and objective actigraphy-derived insomnia symptoms (i.e., total sleep time, sleep onset latency, waking after sleep onset, sleep efficiency) among those randomized to guided Path to Better Sleep, during the first and last intervention weeks. Exploratory Aim 2. Describe the nature (i.e., prompts or reminders to complete modules, help or support using the program and/or completing the modules, other) and duration (i.e., time) of guidance required by participants in the treatment arm to complete Path to Better Sleep.

Identifying DNA Methylation Alterations of Chronic Effects Of Blast and Disturbed Sleep
Role Description: The goal of this study is to use genomic approaches to investigate how exposures to blast result in indelible epigenetic signatures, specifically DNA methylation, which we can then investigate correspondingly in our Veterans. Alterations in DNA methylation are associated with a significant number of physiological and psychological conditions that can affect health and quality of life.

Examining the Effectiveness of an Adaptive Implementation Intervention to Improve Uptake of the VA Suicide Risk Identification Strategy.
Role Description: The goal of the proposed project is to employ a Sequential Multiple Assignment Randomized Trial (SMART) design to improve the implementation of VA Risk ID’s three-stage screening and evaluation process to fidelity (in correct sequence, by the appropriate provider, and within the designated time frame) for Veterans receiving annual screenings for depression and PTSD.

Integrating signals of suicide risk from DoD and VHA data to improve upon suicide risk prevention strategies for combat Veterans
Role Description: The proposed study will be the first to facilitate the merging of large data sets from the Department of Defense (DoD) and Veterans Health Administration (VHA) to identify factors associated with suicide risk. The ability to observe individuals' symptoms and behaviors over time, including during periods of transition (e.g., discharge from the military, transition into VHA care) is expected to inform improved risk assessment and intervention strategies. Knowledge gained will improve the wellbeing of military members and Veterans, and reduce future deaths by suicide.
