    Hillary Lum, MD, PhD conducts patient-centered outcomes research to improve advance care planning for older adults. She is a geriatrician, palliative care physician, and Assistant Professor in the Division of Geriatric Medicine, Department of Medicine, at the University of Colorado School of Medicine and VA Eastern Colorado Geriatrics Research, Education, and Clinical Center (GRECC). Her research focuses on developing and implementing effective models of care that integrate advance care planning into primary care, including a novel Advance Care Planning Group Visit. The Group Visit intervention engages older adults in advance care planning conversations and communicating their preferences with their healthcare system. As a Health and Aging Policy Fellow, Dr. Lum leads the development of a life-span model that include older Coloradans, called the Colorado Opportunity Project. This project identifies meaningful indicators and associated evidence-based interventions that promote well-being and independence for older adults.
